Tell us what you need — medical, psychiatric or both. We’ll match you with the right provider.
Before you arrive, we confirm your plan and let you know exactly what to expect — no billing surprises.
In office Tuesday–Thursday, or by telehealth on Mondays for psychiatry. Doors open at 8 AM.

What to bring to your first appointment
- Photo ID and your insurance card
- A list of current medications and doses
- Records from previous providers, if transferring care
- Your questions — all of them
Arriving a few minutes early helps with paperwork. If anything is hard to gather, don’t stress — call us and we’ll figure it out together.
It’s easier than you think. You’ll get a link before your Monday visit — tap it from a phone, tablet or computer with a camera, find a quiet spot, and your provider does the rest.

In most cases, no — you can call us directly. Some insurance plans have their own referral rules, and we’ll help you check yours when you call.
Availability changes week to week, but as a practice that’s actively welcoming new patients, our goal is to get you in promptly. Call and we’ll offer the earliest fitting time.
Medical care is adult-focused. Psychiatric medication management welcomes patients ages 8 and up with Jordan Lozada, APRN, PMHNP-BC.
