Our Care
Two kinds of care. One caring team.
Heartland brings medical and psychiatric care together, so the people caring for your body and your mind are under the same roof — talking to each other, and to you.
Adult primary and internal medicine with focused care for breathing and sleep — so you can breathe better, sleep better, and live better.

Care for the whole of you

Comprehensive, adult-focused medical care — prevention, diagnosis and ongoing management from a team that knows you.
Focused attention for breathing and lung health, guided by Jana Powell-Sanders, PA-C.
Better days start with better nights. Evaluation and care for sleep concerns and healthy rest.
Careful prescribing, honest conversations and steady follow-up for lasting mental wellness.
See your psychiatric provider from home every Monday — same care, no drive.
Psychiatric medication management for young people ages 8 and older, welcomed with patience and warmth.

Meet Your Care Team
Providers who treat you like a neighbor
Three experienced clinicians, one shared standard of care.
Jana leads the medical side of the practice, caring for adults across internal medicine with a special focus on pulmonary and sleep health. Her approach is simple: listen first, explain clearly, and build a plan you can actually live with.
Michael is a psychiatric-mental health nurse practitioner providing thoughtful medication management. Patients know him for steady follow-up and for treating every visit as a conversation, never a conveyor belt.

Jordan is a board-certified psychiatric-mental health nurse practitioner welcoming new patients ages 8 and older for medication management. She offers telehealth visits on Mondays and sees patients in office Tuesday through Thursday.
Care from the Heart. Support for Life.
We treat every patient the way we’d want our own family treated.
Honest answers, careful medicine and your trust earned at every visit.
The standard we hold ourselves to in every exam room, every day.
